Rates

Individual: My rate is $180 per session. I accept all major credit and debit cards.

Couples: My rate is $200 per session. I accept all major credit and debit cards. 

Payment is expected at the time services are rendered via the HIPAA compliant platform, Simple Practice.  

Insurance

I accept Lyra Insurance. 

I am not in network with other major insurances, but can provide you with a detailed receipt for reimbursement from your insurance as an out-of-network provider. I recommend contacting your insurance provider in advance to learn about the reimbursement process and reimbursement rate for out-of-network mental health providers. Below are some good questions to ask: 

1. Do I have out-of-network mental/behavioral health benefits?

2. Do I have an out-of-network deductible that has to be met first before I get reimbursed? Has any amount of my deductible been covered this year?

3. How much would I get reimbursed?

4. What is the usual and customary rate covered by my insurance for outpatient psychotherapy (CPT code 90834) (This is a specific code insurance uses for billing)?

5. Is approval or pre-authorization required?

6. Is there a limit to the amount of sessions I can have in a year?

7. How do I submit claim forms for reimbursement?

Sliding Scale

I provide limited sliding scale appointments based on availability. Fee is based on income on a case by case and short term basis. Please contact me to learn more information.
